New Patient Registration

registrationWe accept new patients living anywhere within the Practice Area.

If you wish to register at the surgery you will need to complete a new patient registration form. These are available from any NHS surgery or if you contact us by telephone we will send one out to you.

We also ask new patients to complete a questionnaire form which is available to download here:-

When you bring your completed forms back to the surgery you will be asked to make an appointment for a new patient check. These are usually carried out by the Practice Nurse and allow us to obtain some past medical history details from you before your notes come from your previous surgery. The Practice Nurse will also complete a brief health check and add any repeat medications you currently take to our computer system so that you can continue to order your medications in the usual ways. Patients are asked to bring a specimen of urine to their new patient check.

As soon as your details are added to our computer system you will be fully registered at the surgery and then we will request your medical notes from your previous surgery. Please bear in mind that they can in some instances take a few months to arrive!
