Registering with the surgery
To register with the surgery:
- If you are 13 years old or over, fill out a new patient registration (13 and over) form
- If you are 12 years old or under, fill out a new patient registration (child under 13) form
- visit reception from 10am to 6pm and collect a paper registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Changing your contact details
It is important that we have up to date contact details on your medical record.
If you need to update your contact details please get in touch with the surgery.
